First Visit

CP9A8226.jpg

What to Do:  On your First Visit please arrive at your scheduled appointment time to fill out the necessary paperwork and bring any insurance or referral information that is necessary.  If you have any MRIs, X-rays, or any diagnostic testing please bring the actual films if possible but the report can help until we acquire the images.  Also, please wear or bring comfortable clothes (T-shirt, shorts, jogging pants) so you feel comfortable and unrestricted during the examination process.

What to Expect: On your first visit we will take a thorough history and perform a physical and functional examination to help find the source of your pain, injury or disability, even when standard diagnostic tests have not revealed specific problems.  In most cases treatment will begin on the same day unless the nature of the problem makes that prohibitive.  Initial treatment may include activity avoidance, home care exercises, manual therapy, manipulation, laser therapy, dry needling or a combination of these treatments.  This initial exam and treatment will take 30 - 60 minutes so please allow adequate time.

Your Health Coverage

Insurances are confusing but together we will help figure it out.  Here at In Motion we truly try to help you understand what your insurance will and will not cover so there are no surprises.  Things that you can do to help understand better would be - Contact the number on the back of the card and ask the representatives what your chiropractic coverage is, how many visits are allowed per year, is there a limit to how much the insurance company will pay per visit, are there special forms that will be required and do they require a referral.  It is important to remember, if you are using you health insurance - we file that on your behalf with an estimate that the insurance company provides us.  Once we receive the EOB (explanation of benefits) we will then know for certain your financial responsibility.  There are so many different plans available and they change every year - even if your card looks the same as last year - but we will work with you to help you through the maze.

Below are the insurance companies we are Contracted with.  Please call the office for specifics as there are some specific plans within these companies that are closed and we are not In-Network.

  • Aetna

  • Anthem

  • Blue Cross/ Blue Shield
